I don't have a template, but you can do it on a spreadsheet. You can either do it for just one subject area or all content area. You basically have a heading for each month and under each heading you note what you are planning to cover for that month. It's a good tool that each grade level can use to compare what they are teaching and the areas they are overlapping. A good book on the subject is "Mapping the Big Picture: Integrating Curriculum & Assessment K-12" by Heidi Hayes Jacobs. I bought my book from amazon.com. Hope this helps a little
