I couldn't stand to know that bad report was just sitting in my file waiting to bite me in the behind! I'd go to the advisor and fess up also just because I'd want the peace of mind and maybe as another poster suggested it really wasn't yours.

and I agree with asking that teacher for advice on how to be a better teacher IF you can stand to hear what she says and remember not to take it to heart.

Your cooperating teacher may have written your report on a bad day and when she really sits down and talks to you she may want to reconsider what she wrote.
