I agree! Unless your cooperating teacher has been less than supportive, then she realizes that you are still learning the ins and outs of teaching, and I am sure she expects there will be times when things go awry. It is| better to be honest with her than to try to cover it up though. She will respect you more for your honesty than if you were to "sweep it under the rug" so to speak. If you would feel better, offer to create, grade, and record two math assignments that were similar to the ones she anticipated grades for. This will compensate for the missing marks. Also, if you are required to do a self-evaluation, | | maybe note that you would like to improve your record-keeping skills. When you approach it with an "I made a mistake, but here are some ideas for trying to rectify it" attitude, I think your cooperating teacher will respect the fact that you care so much. Admit the mistake and move on. The worst that can happen already has--a few grades are missing, that's all. You will still get through your student teaching experience and learn many things along the way! Good luck to you!
