I made up a form letter, which I know gets a positive response. It basically says:
Here's how the day went
Here are the lessons we completed
Comments/concerns
I always make sure that I list positives as well as negatives under that last heading. I also give more detailed notes on the back if needed. It has a few designs on it, which I made on my computer. It serves as my business card, too.
