I usually just write in the borders of their lesson plan and then a note at the end w/ any other info and my phone nbr. I think a bus. card is better (or 2). I try to go back and meet the teacher after the day if I don;t
know them just to have them know me and to make sure everything went well. And then they say be sure and give me your phone nbr.....Guess the paper got pitched so Bus. cards or 2 are better to leave attached to a form. I like the form idea. In fact at one school last week, they gave me a sub folder that had a similiar form in it for me to fill out and return to the office...the teacher
did see it but I thought it was interesting. Also, I had a badge to wear (not always) and every adult on campus greeted me w/ a smile. Very welcome reception!
