I leave a form that is 2 sided titled "Welcome Back". On this form I list my name, phone number, email address, date. I have typed in italics and underlined Welcome Back! This checklist/form is a quick way to let you know
what happened while you were gone. Thank you for the opportunity to work with your students. Then I have a brief sentence that lets the teacher know if there were any notes or notices and if there were that they are attached. I list absences, helpful students, disruptive students, general classroom behavior on the front side and on the back, I list subjects/assignments/lessons completed
and not completed. After I fill out this form I usually make a copy of it before I leave the school for my records. Hope this helps. I have found it quite useful. Good Luck.
