I'm also a substitute teacher interviewing currently and hoping for a job next Fall, so I can't speak for a "real" teacher, but I also have a form I use. I got 100 sheets of cute paper with a school type border cheap at an office supply store, and wrote something like,

Date
Lessons Not Completed
Behavior

(with space between where I can handwrite the responses)

Then at the bottom, my name and phone number, with a message that says, "Thank you for allowing me into your classroom". I've never heard anything bad about it, and I think that if I were the permanent teacher, I'd like to know that I can reach the sub if I have any questions=)
