    Carrie,
    I teach K-4 severely emotionally disturbed students, so I am doucumenting all of the time. You always want to write down what happened before, during, and after the misbehavior took place. Be very careful not to say things like... the student looked or appeared angry, upset, or sad (unless they told you they were one of those things, you don't know how they felt)nor do you write I think the student....(only write what you know!)If you want to write a comment about how they appeared, say "In my opinion, the student appeared.....
    I keep a checklist of the 4 most common misbehaviors that my students display, and every 30 minutes, I put a check mark beside any of the behaviors that they may have displayed. The checklist starts from the bus ride to school until they get on the bus in the afternoon. This checklist is very useful to see if there is a pattern of time of day when a student misbehaves. It also serves as easy documentation. I hope this is helpful and not too confusing. Good luck with your class!!!!!!!
