The Grade 3 Mailbox Superbook has a template for the idea you are talking about. I am using it with my kids. If you don't have access to the book, I'd be willing to copy that page for you and send it to you. The book is not cheap...but it is full of good ideas. I think it was around $30. I am new to grade three this year so I felt I needed a "superbook" to help me adjust to my new grade level.
