Sorry I am so late in replying. Lots has been going on in the last couple of months and I've just not had the time to look at this board.

In Word, there are 2 toolbars that you need showing to create fill-in templates.
Click
on the View menu and move down to Toolbars.
Make sure to check Forms toolbar and Control Toolbox toolbar. These will allow you to add fields and check boxes you can fill in and a whole lot more. You can change the properties of the fields to have them act different ways.

When you get everything the way you like it you can click on Tools and Protect. Be sure "forms" is selected.

You can key in a password and protect the form from being changed. Just be sure you can remember the password.

When you save the template, be sure to save the filename with the .dot extension. This marks it as a template. Also, save in the templates folder of MSOffice so it will come up with the rest of the templates.

If you save it with .doc extension, you can still use it, but you will have to put it where you can find it, open it and immeadiately rename it so you do not save your added text to the document.
