While it could be a sign of abuse it also could be irritation from the brand of soap his mother uses...You might talk with his mother and suggest that she get a doctor to check him. Little girls get irritations from soap, why can't little fellows?
There are many aboveboard explanations.
Also, several years ago I had a second grader come into the room after lunch with both hands in his pants. I scolded him and he continued to do this. I sent him to the office for being defient. Just imagine how small I felt when he fell out about twenty feet from my room with a seizure...his first ever. that was what was going on when I was after him for not answering me!
