i'm assuming you mean classroom dis. policy.
here is mine:

there are 2 rules in my classroom:
1. be responsible
2. be respectful

the first day of school, the students brainstorm positive examples of these and
we write these below the 2 rules. (there are certain things i like them to always list, and i guide them there if they do go in that direction. ie: raise your hand to speak, stay on-task)

i stress that students create their own consequences from their actions. my list of positive consequences are:
special helpers
caughtyas
star cards (stamps for on-task)
treats (candy)
newbucks

(fake $ for turning stuff in early)
notes to students or parents
all-work-in parties (for all work in for 2 months)
phone calls to parents

these things are done randomly for appropriate behavior, attitude, and work.

negative consequences:
1st time in trouble in one day--warning
2nd--time out (side of room, or in hall, and fill out a behavior form--what i did wrong, why i did it, what i'll do next time....)
3rd--school service (clean up, file....)
I'VE ONLY HAD TO GIVE OUT THE 3RD CONSQ. 2 OR 3X.
4th--conference with teachers (we rotate our kids through 3 teachers), parents, and principal
I'VE NEVER HAD TO DO THIS LAST ONE
