Hi Laura,
Here's the outline for my Reading Methods Case Study Project. Though the assessments will differ, the ideas within each topic should be useful. If you need additional help, just post or email. ;-D

Introduce your child with a brief paragraph. Do not use the child’s last name.

Summary, Recommendations and Comments (Should be at least one page in length.)
What can you say about the child’s literacy development? Be specific. Use information gathered during the assessments. What are the child’s strengths and next learning steps?
What are your recommendations? Use what you have learned in class. Be specific. What mini-lessons, needs, suggestions would help this child? See the Language Arts Standards for ideas.

Reflections:
When would it be appropriate to follow up on particular assessments? As a literacy teacher, what have I learned from this assignment? How valuable was this activity? Why is on-going assessment / evaluation important? How does this experience fit my beliefs about reading and literacy?

Please attach all supporting testing documentation with your report. It must be typed. The report should be approximately 3-5 pages in length.
