I'd tell them

that I was going to be having surgery. I wouldn't be specific but the kids will want to know why there's a sub and when you'll be back. Plus kids love to make cards for their teacher and you'll love getting them when you're feeling crummy! If I was a parent in your room I'd want to know that while you were going to be gone that a competent person would be there for you....so I'd send home a generic note if you don't really want them to know what the surgery is for don't say.... If you don't care then tell them. At any rate they all deserve to know you're going to be gone.
