The DRA is what you need...find it at Celebration Press, which is a company that Pearson Learning now owns. In addition to having a complete leveled book to read, you take a running record, and evaluate the child's retelling of the story. There is a comprehension rubric and affect/motivation questions. Once you've done a few, you can adapt this to almost any text and genre. Best of luck, Dominie Press also has an assessment using real leveled stories, which was developed by Carol Lyons (a reading researcher). ;-D
