I got this idea from a teacher I substituted for one year. It's called an "OOPS!" book in my room. I have the word printed in big orange letters on a black binder. Since I teach three sets of students for science, there is a tab for each homeroom, with notebook pages following. Once a student has received a warning for an offense, a page is created for them in the book. Their name goes at the top and on the line along the right side of the page. Once their name is entered, I cut the right column to the space below their name. This creates an instant tab, so I can scroll the book to find their name without worrying about them being alphabetized. It's quick, easy, and extremely effective, particularly at parent conferences.
