Discipline Log

I use a Discipline Log with my 5th graders. They have to sign their name, put the date, write the number of the rule that was broken and then write an explanation of what they did. My rules are very general so I make them write a more specific explanation so I can recall what happened. The only problem I've had with mine is some of the students are not completely honest in their explanations. I use the same kind of system to track late homework. It works great!
