The system we use is similar - I laminated a class list with columns labeled for different kinds of balls, jumpropes, etc. All of the equipment fits in a plywood box on wheels. Every week a different "equipment monitor" is
responsible for rolling the box outside the door in the morning and back in at the end of the day.

S/he also checks out equipment as students walk out the door at recess and checks it back in as the students return to class at the end of recess. (The list is on a clipboard with an overhead projector pen attached to it.) Students who do not return their equipment cannot check out anything

else until they find and return it.

Most years this has worked pretty well! One more thing - the monitor is responsible for making sure all of our equipment is clearly marked with our room number. (We keep a good marking pen in the bottom of the box.) The students watch for missing equipment and return it to our box.
