Hi Crystal. If I were you I would decide which behaviors seem to be most disruptive to the student's learning or of the other students' (perhaps that she cannot remain seated or talks out) Are there any particular activities
these occur most- perhaps size of group or type of activity. Perhaps the behaviors are less during one on one work or during active activities. The observation notes you take will help you identify this.

Once you have identified the behavior you want to improved and possible reasons for the behavior (perhaps a need for attention, does not understand work, poor concentration)

your plan will help address the reasons causing the behavior.

Here are some examples:
Teacher attention contigent on completion of a task
Placing within small rather than large group for instruction
Seated close to teacher
Visual reminder of appropriate way to ask for attention.

Visual cues for task completion
Seated in quiet area, less distractions, close to teacher
Use of stickers/stamps on a card to earn reward
Break down tasks into very small steps with teacher attention after each is completed
