I do a similar system to Cheryl's. Each student starts on green for the day. Each "warning or reminder" is a change of color.
Green = great day
Yellow = Ooops, warning
Blue = 5 minutes of recess with head down on desk
Oragne = 15 (all) of recess with head down
Red = Warning slip sent home & must be returned the next day

This works. There is a lot of different systems to keep track of their colors (on a class chart or on individual desks).At first I was giving rewards for students who stayed on green all day & then eventually stopped that. I often times moved them back a color if they were showing improvment. This is a very popular system I'm sure you've heard about.
