Try the book, Cognitively Guided Instruction...It is fascinating and comes with two CDroms to show the children in actual problem solving activities. In training class, we got a CGI interview to do with each child in our classroom. A bit time consuming as it's 1:1 as is the Ob. Survey by Clay, but well worth the valuable information you learn about each child. It's jointly published by NCTM (National Council for Teachers of Mathematics) and Heinemann.
