Sherie, I just finished my intership year, and saw many different teachers who implemented portfolios in many different ways. Some of the teachers used the subject based portfolios, some used an all purpose portfolio,| and some even used electronic portfolios (those were the coolest)! As for how often, I saw one teacher do it this way: On the last part of the day on Friday's while students were finishing work or reading, she would have portfolio conferences. After saving ALL work that the child had completed the entire week(either in a work folder or in those cardboard shelving systems with labels | | for each child), the student and teacher would have a short conference at the desk to decide which pieces of work should be included in the portfolio. The student was allowed to choose one, and the teacher was allowed to choose one. Both the student and teacher have to explain why they are choosing this piece. Of course, sometimes more than one piece of work was chosen. They actually filled out paperwork on each piece, that had title, reason picked, and some other things. I actually have copies of those forms if you'd like them, just send me an email. anyways, just a suggestion. Good luck with portfolios. This is a wonderful authentic assessment, and I think all teachers should implement it in some form.
