In our school we are not allowed to send out a generic form at the beginning of the year for bus trips. We can send one out for walking trips in our area.

So for each field trip we send out a form. I make my own, but
our office also has a generic form.
Generally my form looks something like this.

Field Trip Permission Slip
Dear Parents and Guardians:
On Monday, June 9th the grade one class will be taking a field trip to the Swimming Pool. We will be leaving the school at 9:30 a.m. and returning at 3:00 p.m.
Lunch will be provided.
Students will need to bring a swim suit, towel and

quarters for a locker. They may also bring money to buy goggles or to rent a life jacket.

Please sign the form below to give your child permission to attend this field trip.

My signature

(name of child) has permission to attend the field trip to the swimming pool on Monday, June 9th.
(Signature of parent or guardian)

Contact Number in case of emergency:

I am able to attend this field trip as a parent volunteer (box to check)

Where the student's name is and the parent's signature I have lines drawn.

I also include at the top of the page, the name of our school (or even our letter head design).
