I also keep a monitoring notebook with divided sections for each child. In addition to anecdotal notes, I keep all their running records, problem solving interviews, etc. I use a variety of check sheets in different subjects, ex: one for the cooperative learning / process stuff...Headings include things like: attends during mini lessons, gathers materials for problem solving, ventures to set up the problem, discusses possibilites with partner/group, etc. Hope this helps. ;-D
