Bathroom breaks

I don't know if my suggestion will help you, but you can try it if you want to. We take two bathroom breaks each day, one at about 10:30 and the other after lunch. Other than the class bathroom breaks, the students are only supposed to go for emergencies. This was happening very frequently so I gave each student a laminated piece of construction paper with their name and bathroom pass written on it. If they have an emergency, they have to turn in their pass and sign out. Once the pass is used that's it for the day. I wondered if this was going to mean every student would try to get one extra bathroom break in or if it would really solve the problem, but I only get the passes from a few students each day. It seems to work so far.
