To all of you patiently waiting for the template - doofus (and busy person) that I am, I again forgot to bring the template home. I will get it tomorrow and get it out to your Saturday a.m. The author is Margaret Wise Brown. The book was rereleased in 1999 with some wonderful illustrations. I checked on Amazon.com - listed at $5.95 (hardback).
