Our school has a website and they send home a permission slip to each child's parent requesting permission just as you stated it. You could easily creat a form with a YES, my child, _______, has permission to have their picture and first name ONLY posted on the website; or NO, my child...I would also give a little explanantion of what you plan to do with the website and the address for parents to visit. If they have any questions, concerns, blah, blah...You know the routine. I would send you our form, but it is a part of several forms all in one. Good luck with the website!
