In our district this paper is called a "disclosure document" and is required of all teachers. The specifics for what needs to be included are vague. It would be very helpful for you to see what other teachers at your school| hand out. If at all possible you should contact some of them (especially on or near your grade level) and ask for a copy of their information. In general, you should include: Texts and other materials you will be using in each subject. It's perfectly ok to indicate "manipulatives" or "hands-on materials" rather than to get more specific. A statement to the effect that students | | are expected to learn subjects required by your school board or district and that you will be teaching that material. You can list general expectations for math, reading, social studies and science. An example would be to say, "In math we will be learning counting to 100, addition and subtraction to sums of 20, and simple measurement." Or whatever is appropriate for your grade level. A short statement about how students will be evaluated and what will happen if a student is failing. A tentative schedule of activities on a typical day. You can always change this later when you've had a chance to fine-tune things in your room. You might include a sentence or two about your philosophy of education, a request for parent involvement or suggestions for parent/child activities to enhance classroom work. Our disclosure is about 2 pages long.
