    I have done it in the past using Excel and copying and pasting it into Word. You would format the lines like you do a table: I use a bold line on the top and bottom and a dashed line in the middle. My pet peeve: my workbooks don't have midlines. Second graders need midlines.

    If you need more detailed directions, let me know. I'll go back into Excel and figure it out step by step.
