Excuse Slips

I've made small "Excuse Slips" to give out to students who have not completed an assigment. On the slip the student writes their name, the date, the assignment, and the reason why the assignment was not completed. (Some of the reasons are hilarious!) I then tape that to the filing cabinet next to my desk. Upon completion of the assignment I file the excuse slip in a 3x5 box on my desk. At the end of each quarter I clip the excuse slips onto the students' report cards for the parents to see. (Of course, if the student is chronically late with assignments I don't wait until the end of the quarter. I will make phone calls prior to the end of the quarter). This has worked for me.
