I'm not sure this is going to help, but for me, it depends on the parent. For most, it is the formal name and I always start with that. But over the years, I have become good friends with a lot pf parents at my school, or I have had their older kids. If I already have a relationship with them, I usually call them by their first name and have them call me by my first name in return. Most of my parents are older than I am, so starting out formally is my best bet. I try to create an environment where we are all on the same page, and by the end of the year I am on a first name basis with almost all of the parents.
