I had a reading log that had the title/author and number of minutes. There was a space for parent initials...i did away with this halfway through the year and started requiring Literary Letters. I did this because kids weren't turning in the logs. This way it was like a mini book report each week and held the kids more accountable for the reading. I require 30 min a night in 4th grade.
