We don't have that requirement. We do have to fill out a self-evaluation at the beginning and end of the school year, and we do have something like that. It asks a question about what kind of professional development you did over the summer and how you plan to implement what you learned. Then at the end it does ask for some examples of things you did that were successful. But it's pretty informal, and year before last our principal forgot to ask for the end of the year one to be turned in!
