I keep it simple..

When the kids take a test they record thier score in thier data folder on a graph. I do not grade them or keep track of it at all.

Each month if student's get at least 3 points they are invited to "eat a book". This
is when I read a story to them and they get a snack that goes along with the story. This month I am doing "Diary of a Worm"and students will get gummy worms and pretzel sticks. I have also read a book about camping and given out smores. I have tons of books/snacks if you are interested in this idea.

This type of reward usually motivates them enough and I don't want to spend
tons of my time keep track of something like this...I don't really think the scoring is accurate for many reasons-such as if the kid doesn't read the book and just guessing they can usually get a decent score.
