I send a similar survey to my parents at the end of each school year. It really is helpful. I tend to look at them more closely when I go back to school in August as I am making plans for the new year. I did have one parent who was unhappy all year and I expected her responses to be somewhat negative. You take it with a grain of salt. Most responses are very helpful.
