Where we live, IB is a school within a school. My daughter is enrolled in IB, and the school itself is about 1000 students, but less than 200 of them are enrolled in IB. The freshman class starts out at 50, but at the end of my daughter's junior year, the class has 35 students.

I think IB teachers need IB training/certification. Everything is paced at a more advanced level. My daughter was using a college level textbook in her freshman European History class and took the AP test. She's taken an AP test each year. They do take state testing, but since they're teaching and learning is way past the state minimums, they're very casual about it. (Oh, by the way, our SOLS are day after tomorrow.)
