I failed a student teacher 2 years ago. Many similar problems to yours. Do not pay attention to the slamming post. I carried some guilt for this ST failing, but reading your posts I realized that the empathy and support I feel for you could be applied to myself as well. Reading your posts has made it easier for me to let go of the guilt I felt. My ST last year was AWESOME. Don't give up on guiding future educators!
