are you sure she is supposed to evaluate your teaching? i would think her evaluation of you would be more along the lines of how you did as a mentor teacher...i.e:

mentor teacher answered my questions in a timely manner
mentor teacher gave constructive criticism in a timely manner and out of earshot of others
mentor teacher gave me encouragement and postive feedback
etc.
