Is she mentally challenged or mentally ill? Makes a difference. If she is mentally challenged she may not be able to
1. help the students
2. be proactive
3. remember routines from one day to the
next
4. independently supervise a classroom of children

She may be operating at the maximum already.

It is hard to offer advice because I am not sure what you want to accomplish with this evaluation. Do you want her
1. to be reassign
2. to lose her job
3. to improve her performance
4. to work independently
5. to be a self starter

If you think
she is capable of doing a better job, what direction have you given her? training? reinforcement for doing something right? correction for doing something wrong?

I really would like to offer suggestions regarding the evaluation, but without knowing more, I would probably be giving you some misdirection.

If you don't think you can work with her next year, talk to your principal.
