Here are a few of my favorite sites:
http://www.gigglepoetry.com/
http://www.ettcweb.lr.k12.nj.us/forms/newpoem.htm
I also really like the interactive forms on read write and think

I have my students take turns taking home a ziplock bag with 3 paperbakc poetry books inside. They need to pick a poem, practice it so it cqn be read with expression and then bring the poetry pack home and share their poem with the class. I have 3 packs out at a time so I have the kids bring it back ready in a few days. My kids like it so much that we usually don't want to quit at the end of April and I have to cut it off in late May so I make sure I get my books back.
