i'll second what cvt said---as a 6th grade teacher, i have often been the final straw that breaks the camel's back. for one, the parents have FINALLY heard it enough times---they can't really blame it on the teacher at this
point, since it's been so many teachers. also, i have this thing called jr. high i can hold over their heads. it's going to be so much harder and busier next year; we really need to get him to concentrate.

perhaps you could walk over to him and break his assignment up into increments. "your job is to do this one problem within the next 2 minutes; raise your hand when you're
done." then go to him when he raises his hand, praise him and then say "now your job is to do THIS one in the next 2 minutes", etc. (you might need to give him an individual timer.

(now, if he CANT do the problem, you say "you have the next 2 minutes to either do this problem or raise your hand to get help with it")
