The forms that the Mariely submitted are wornderful and I will definetly use.

In addition, I have the students write down what happened right after an incident resulting in time-out, or detention. It works out
really well because I don't always have the time to document every incident as soon as it occurrs. Students "write their statements" including when they don't bring in homework or complete assignments. They are required to explain why there is missing/incomplete work or any incident that has happened. Students who have witnessed also complete slips.

The slips add up, however,
it is a great way for me to recall facts at the end of the day or week. It is also a wonderful way to have the students voice their side of a story. I also use the slips as proof of certain facts and events for parents. Especially when I hear, "My child was doing his/her homework last night." I keep the incident slips organized in a binder with pockets. I don't even have to keep them organized myself, because I train a trustworthy student to file them in the proper place.
