Most the time my documentation is just a post-it note with the date and a short note so that I can refer to it later. Same thing with parent phone calls. I have a file for each student and stick notes onto their registration information. When I get notes from the office or from parents I write the time I called them back on the note and file it, or if I write them a note I use a carbon copy, date it and stick the copy in the child's folder. I bet there is a classroom management book somewhere that has some standard forms in it for documentation though.
