who initiates

Thank you for all your feedback, however, I don't think I worded my question right. Who initiates them, does observations, etc? When we actually put the plan into action it is done with the entire team including the parents. Years previous, our psych did most of the background work including paperwork and we contributed in a meeting and all were responsible for implementation. This year we have a new psych and I am doing all of the leg work prior to the meeting. I don't mind doing this. I was just curious how it was done at other schools.
