An idea for discipline

I'll have LD, not any ED that I know of. Here is what I thought of the other day. I'd appreciate any "constructive" feedback. I will have five rules posted and sent home. I will go over these rules with the students. First offense: verbal warning
Second offense: student must fill in a behavior form I have that I will keep in a notebook. (Student writes the rule broken, what else he could have done, why he did it, etc.)
Third offense: behavior form is sent home and msut be signed by a parent Any major infractions will warrant a phone call home. I'll also give praise and stickers or other rewards for good behavior and homework. How does this sound?
