Success for All reading program that is similar to what you are describing. I taught at a school that used it for 3 years. It was a success for our school but it takes about three years to see the long term progress. How
it worked was at 8:20 am a song went on the intercom to let everyone know it was time to move to their classes. WE just did k-5. Students were tested and placed in the appropriate classes trying not to keep older students grouped with similar age groups but reading on the same grade level.

Every nine weeks the child is tested again via computer test and placed based on their new
test scores. Some moved and some stayed in the same reading levels.

It was very scripted but now that I teach guided reading I can greatly appreciate that program and most importantly it really did work. You are no longer pulling from every which way materials K, 1, 2, 3, 4, 5, to accomodate the many different reading levels in your classroom. I wish our school would go towards something like this.
