ask the nurse to visit with them privately about their teeth and the issues this can cause for the dentist -one of which is braces. Then develop a secret signal like tweaking your ear etc. which reminds them they are doing it. It's going to be hard for them after this length of time. It's also a way to relieve anxiety which will then pop up in some other form.
