I don't know, but here's a thought

I'm sure you have tried ignoring this behavior by not making eye contact with him and asking others not to react. Sometimes no reaction will eventually extinguish the behavior. I have never experienced this before so I don't really have any suggestions that I know would work, but I was wondering if it would be feasible to give him something like a small squeezable ball and tell him that whenever he feels like saying those words he could squeeze the ball instead? I don't know and I'm really reaching here, but maybe it would help him to focus his attention elsewhere.
