I use the following attachment. It is for a HW grade. Here is how I grade it: 20 points for each day it's done, so if you do your HW every day you can get an easy 100. It's up to them and the parents to make sure this is done and I check it daily. HW only counts as 20% of the subject grade, so it hleps boost their grade, but not that much.
