have him screened or tested for OT? If he's still writing like that despite instruction, I would guess he could be a candidate. Other things that are red flags (alone or together) for that are:

- cutting is very frustrating
and messy
- drawing looks "below" grade level
- has trouble picking small things up off of a table
- plays lots of large motor sports

Things you can do while you're waiting:

- try raising up his papers by having him write on an empty thick three ring binder, or if you have a very open classroom where other kids wouldn't notice: magnet his paper to a vertical
surface and have him write like that
- have him use a pencil grip
- ask his mom to have him play with play dough 3-4 times a week, concentrating on making things with the tips of his fingers, not palms- emphasis on pinching
- have him practice "inchworming" up and down his pencil with one hand
- have him practice taking small objects (like little erasers) out of a container filled with small macaroni and then "squirrel" it into the palm of his hand (using one hand only)
-relieve him of writing on three lined paper for a while while he is getting this worked out. I have a kiddo this year who wrote exactly as you said, and just letting him write on one line straightened everything out. He now can write on three.
